We are pleased to introduce the Heart To Heart series, which aims to address some of the key challenges and concerns faced by community-based heart failure specialist nurses (HFSNs) in the UK.

The series contains videos from HFSNs who share their experiences in overcoming various challenges (such as working alone and lack of familiarity with managing patients in a virtual setting) in order to support nurses who are facing similar situations in practice.

You will also be able to download summary documents from each video below with suggestions on how you can achieve solutions to challenges you face in your local area, and in turn help to more effectively care for your patients with chronic heart failure with reduced ejection fraction.
